With our medical aesthetics practice, we needed software that controlled inventory overhead, clinical compliance profiles, and commission structures. As a med spa, we started seeing two platforms—PatientNow and Boulevard—as dominant choices. I found, researching both, that they approach practice management from fundamentally different philosophies.
Bottom Line Up Front
PatientNow is an industry workhorse built from the clinical electronic medical record (EMR) outward.
Boulevard is a modern platform designed from the consumer calendar and retail point-of-sale outward with one of the best interfaces I’ve ever seen.
Let’s see how they compare.
Core Audience

Obviously I want to know who the core audiences for each platform are because I need to ensure that it aligns with our primary business purposes.
PatientNow
So, PatientNow is designed for multi-provider medical spas, plastic surgery centers, dermatology clinics, and multi-location elective medical enterprises. Basically, any business that performs elective aesthetic procedures. Their architecture is built to support plastic surgeons, dermatologists, and advanced nurse injectors, with a platform engineered around HIPAA compliance, legal liability protection, and structured data entry.
Boulevard
If, like us, you offer some of these services, but not all, then Boulevard is a better pick because of its streamlined interface, additional features (without the additional costs), and easy learning curve for staff. Boulevard is there for high-end beauty, salon, and luxury hospitality companies, built as a “Client Experience Platform.” This means Boulevard is best suited for high-end wellness boutiques, premium single-location medspas, and scaling aesthetic brands.
Services

Just as important are the services that each platform can support.
PatientNow
PatientNow has a lot of deep clinical services and extensive internal automation. At its core, you get a heavy-duty EMR, treatment charting, standardized medical consent forms, and automated pre- and post-care patient communication. You also get tools for electronic prescribing (eRx), direct lab ordering integrations, and ICD-10 medical coding modules.
If your med spa or practice has on-site surgical interventions or medical weight loss services, they give you a complete framework for internal support. Pretty useful.
Boulevard
Boulevard’s service suite focuses on maximizing front-office efficiency, appointment density, and retail commercial velocity. So it is a better fit for businesses that offer some med spa services, but perhaps don’t focus exclusively on med spa injectables or medical weight loss.
Boulevard’s core features include a single-page online self-booking portal, automated appointment reminders, and a robust point-of-sale (POS) setup. Boulevard supports recurring membership enrollments, gift card management, and front-desk retail upselling too, better for facilities or businesses that generate revenue primarily from memberships and product sales.
Integration

If I decide to implement new software I need to know how well it will integrate with any existing platforms we use, and what the onboarding looks like.
PatientNow
PatientNow functions primarily as a self-contained, all-in-one ecosystem. They have actually scaled to include RxPhoto for advanced photography management and EnvisionNow for cloud operations. Basically you get a unified feature set, but the downside is you have to use all of the internal features because it’s not going to integrate well with other external software.
Time to say goodbye to other business support structures.
Boulevard
Boulevard absolutely took the cake here as one of the most user friendly platforms I’ve ever seen. Everything can be customized and linked to external marketing engines, custom websites, or third-party intelligence tools. They have a direct, native sync with QuickBooks, and you get a real person to walk you through your onboarding process over the span of several weeks, not hours.
Clinical First vs. Client Experience First

I prioritized a platform that would give me a client-first experience, something that was built outward for the client but I know that not all med spas are like that and some want a system that’s set up a little bit differently.
PatientNow
PatientNow is a clinical-first architecture. It is built from the chart outward. What that means is when a provider opens the system in the treatment room, the software provides a medically optimized environment.
The standout clinical feature is its integration with RxPhoto, which supports clinical visualization. Injectors can capture photos from an iPad, apply precise camera ghosting overlays to match baseline angles, and lock those visual assets directly to the patient’s medical record alongside signed consent forms. This ensures an ironclad, legally protected documentation loop.
Boulevard
On the other hand, Boulevard is a client-experience-first architecture. It is built from the calendar outward. So if scheduling is your primary asset, and you want to ensure that everything is done quickly and effectively, Boulevard is great.
The standout feature is the Precision Scheduling feature which has a back-end algorithm to analyze all of the services you offer and automatically shift them into the right order so that when a customer checks out, everything feels seamless, but they don’t end up with a massage booked right after a facial.
Suitability for Small or Large Practices

We started as a single med spa, but as our practice grew and we acquired new inventory, clients, and staff, I had to make sure that what we were using was suitable for our practice not just in terms of size but in terms of location.
PatientNow
PatientNow is best suited for large, high-volume, injector-heavy practices. Basically, large practices with a mix of MDs, Nurse Practitioners, and RNs.
I honestly wouldn’t recommend it for smaller practices. It’s just genuinely too dense with far too many nested menus and a steep learning curve that doesn’t jive with a smaller business. It will, and I promise you this, feel like far too much software if you have a boutique med spa.
Boulevard
Boulevard, however, is an exceptional fit for small to mid-sized practices. If you want to prioritize rapid front desk services and customer self booking with an intuitive user interface, this is it. It excels when it comes to facilitating small team members and increased customer retention.
Suitability for One or Multiple Locations

As I said, the size wasn’t the only consideration; we also had to look at what would happen if we expanded to multiple locations (which we eventually did).
PatientNow
PatientNow is all in for multi-location enterprise models and corporate-backed medspa chains. That’s kind of what their architecture was engineered to support. So if you have multiple locations, PatientNow can push things like synchronized patient data or patient charts across multiple locations.
Basically, if a patient visits Location A for an injectable and Location B for a laser treatment, their clinical histories, treatment maps, and RxPhoto timelines sync across the entire ecosystem.
Yes please.
Boulevard
Conversely, Boulevard performs exceptionally well for single-location boutiques. Their platform can certainly support multi-location setups with a centralized Medical database, but I would genuinely recommend PatientNow for corporate, large-scale locations simply because it takes a lot of administrative oversight to try and add services or clinical updates across multiple locations.
Pricing Structure Differences

Then of course I also need to know how much things are going to cost because not every optimized platform, feature, or tool is going to be within our budget.
PatientNow
I’ll be honest, I don’t appreciate when companies hide their pricing. It always puts me off whatever service they have and unfortunately PatientNow is one of those companies, requiring that you go through the mandatory sales demonstrations and get a custom enterprise quote instead of just giving you a flat rate pricing structure.
Hard pass.
That said, having gone through all of that rigmarole, the baseline monthly subscription package is around $400 but as you scale and you want to include things like RxPhoto platforms or deep compliance toolkits, it can be upwards of $800 per month, per location.
Boulevard
Boulevard, thank the medical gods, is open and honest about their pricing. They have a tiered monthly subscription framework based on the features you choose and how many locations you have. The basic subscription package is around $160 per month but as a MedSpa you’ll probably have to add the Aesthetics bundle which brings you up to around $400 per month, basically the same as PatientNow.
From there though you can add things like custom digital medical charts for an extra $50 per month, QuickBooks for another 50, and so on.
Real-World Gaps

Finally, there were real world gaps that we experienced when using both platforms.
What PatientNow Users Miss
If you switch to PatientNow, be advised that you will lose that modern self-booking interface, Boulevard’s clean digital aesthetic for patients, and the fluid checkout options. You’ll probably struggle with the same thing myself and several of our employees did: click fatigue. It’s just so exhausting to have to click through multiple layers of drop-downs just to clear through a routine chart.
Plus, PatientNow’s patient portal means new clients get shoved through a structured registration wall just to look at open appointments. In our experience, that means a lot more clients getting frustrated and leaving before they finalize their booking.
What Boulevard Users Miss
Now, if you switch to Boulevard for its sleek design you’ll still face downsides like the administrative wall with regard to clinical depth. PatientNow has a pretty impressive aesthetic medical record infrastructure but Boulevard lacks the precision camera ghosting tools, side-by-side comparison builders, and legal photo-watermarking logs. It’s not the end of the world, it just makes long-term clinical outcomes harder to track and there isn’t a built-in electronic prescription option so that has to be managed externally.
Final Recommendation

Both of these platforms can be a suitable option depending on your needs.
- Choose PatientNow if: Your business revenue is heavily driven by medical injectables, OR you have a multi-location enterprise facing strict regional medical board oversight.
- Choose Boulevard if: Your aesthetic clinic operates like a premium, luxury wellness boutique OR a friction-free online booking flow is a top business priority.
