Vagaro is one of the most affordable med spa platforms, so it’s worth exploring for newer clinics or solo providers. The base plan runs $30/month for a single provider at a single location, with HIPAA compliance available at any tier without requiring a plan upgrade. I also love that you set up the BAA directly from your dashboard settings, which is surprisingly straightforward for a platform that very much was not originally designed for med spas.
Just be aware that most of the clinical functionality lives in the Forms add-on ($10/month), which includes SOAP notes, charting, and annotated before/after photo capability. E-prescribing will also cost an additional $40/month per prescriber. All told, the realistic price tag for a solo med spa practice comes to around $80/month.
Being a budget pick, it also has its fair share of limitations. For example, the image markup tool is incredibly basic. Injection mapping is tricky at best, and you can’t edit your notations once you save them.
However, for a solo injector who’s just launching or not ready to commit to a more in-depth system, Vagaro covers the fundamentals reasonably well and at a great price. So go in knowing it’s a starting point for all-in-one platforms, not necessarily the absolute best.

Mangomint is well-known in the beauty space for its high-quality user interface. It’s consistently rated among the cleanest, most intuitive platforms available (for both staff and clients), which makes a huge difference when your staff is managing complex schedules under pressure.
However, there’s a major caveat here: HIPAA compliance and BAA access are only available on the Standard and Unlimited tiers, not on the base Essentials plan. Furthermore, Forms and Charting is a paid add-on that tacks on an additional $50 to the base $245–$295. If you have multiple locations, the price scales up accordingly.
Also, as much as I love Mangomint, it simply doesn’t offer e-prescribing on any tier. So it’s not a true EMR system and may not be appropriate if you want something with all the bells and whistles.
Finally, keep in mind that all customer support runs through chat with no published phone number for urgent issues. They do typically respond within six minutes, but personally, I’d rather call someone when I’m dealing with a crisis.

PatientNow: Best for Clinics That Want AI to Handle the Details

PatientNow focuses more on the clinical side: electronic medical records, HIPAA-compliant charting, and clinical-grade before/after photo management linked directly to patient records with full markup capability.
But they’ve also recently added several AI tools that you pay for separately from the main platform, including a receptionist that handles intake (using voice cloning to match your staff), a sales director that follows up on leads, and a webchat tool that books appointments from your website.
For a practice that needs real EMR infrastructure and is ready to let AI do the heavy lifting, the feature set is very well-aligned.
With anything AI, make sure to ask who’s liable if the tool does inadvertently violate HIPAA — and get it in writing.
You’ll also need to book a demo to get pricing, which isn’t published anywhere.

True to its name, AestheticsPro is purpose-built for aesthetic practices. It includes customizable EMR forms, e-prescribing through an MDToolbox integration, HIPAA-compliant telemedicine via Zoom, injection tracking, SOAP notes, and consent management. For a practice with a diverse service menu (especially one that’s running 50/50 in-person and telemedicine appointments), it has just about everything you might need.
Pricing runs from $160/month for Pro-Plus (two users) to $285/month for Executive (unlimited users) and $350/month for Enterprise. However, I haven’t been able to get a straight answer on what they charge for e-prescribing. The pricing page says this feature may be charged as an add-on, so be sure to confirm that cost during your demo.
Also, be warned that the software itself is a little clunky. It’s hard to remember where certain features are, and they never seem to be where you’d intuitively expect.
Pros:

The platform is split into two distinct offerings: Zenoti OS for core operations, and a separate AI Workforce package covering agentic receptionists, photo managers, and scribes that generate charts from recorded sessions. This comes with the same caveat as PatientNow: get confirmation of their HIPAA compliance in writing so that you aren’t potentially liable.
All told, Zenoti provides quite a bit more features than a single-location or even a two-location med spa realistically needs. But once you’ve got practices in multiple states across the U.S., it’s worth a look. Just note that it requires you to book a demo to get exact pricing.

Boulevard originally made a name for itself in salons and spas. However, over the past few years, the platform has taken major steps to become a true med spa EMR software, including adding e-prescribing as an add-on.
They also offer med spa plans that are entirely separate from Boulevard’s salon pricing and features. The Aesthetics Starter Bundle runs $364/month and includes forms, charts, photo markup, supervisor sign-off, and inventory management. A single $45 provider subscription also unlocks operational access for all support staff, who can manage prescriptions without the ability to edit them independently. It’s definitely on the higher end, but you’d be hard-pressed to find a more complete feature set at a lower price.
What I love most is that the native photo manager gives every client profile a Gallery tab with a side-by-side comparison tool, letting you pull any two photos for that client without a third-party app.
HIPAA compliance and BAA access are included on all tiers as well, backed by AWS hosting and Level 1 PCI compliance.
For any scaling med spa that’s currently logging into four different platforms to manage one client’s journey, Boulevard would be my top recommendation.
