So, when my clients started finding PatientNow a problem for scheduling an appointment, so did I. 

PatientNow has a lot of genuine strengths worth mentioning. They have a robust lead acquisition setup, automated retention tools, good visual tooling, and an all-in-one framework. But the platform can suffer from a disjointed user experience, and it can also be frustrating to finalize client checkouts.

Below, let’s walk through some of the top PatientNow alternatives.

I’ll break down each of these below, as well as when to consider each for your business. 

1. Boulevard: Top Pick for Client-Focus and a Luxury Experience 

Boulevard was designed by people in the industry who looked at the most common annoyances they struggled with themselves.

The platform facilitates simple but effective scheduling, particularly for Med spas or other aesthetic companies with multiple independent contractors working for them, providing different services. The medical charting architecture can be a little bit challenging, however, if your offerings include medical weight loss programs. But beyond that, the online booking flow is highly polished, works well using a computer or a mobile device, and you’ll get some of the most robust client notes I’ve seen in a competitive platform like this. 

Quite literally anywhere you see a client name, you can click and have a pop-up of that comprehensive client profile, including all HIPAA-compliant notes generated in the bottom right-hand corner. From the same area, you can easily process payments, add memberships, schedule new appointments, or even contact clients directly.

You also get the AI Precision scheduling function, which eliminates calendar gaps, makes sure that treatment rooms are used as effectively as possible, and puts bookings in the right service order so that when a client book something like a sauna, a facial, and an eyebrow wax, they don’t end up doing them in that order. 

2. AestheticRecord: Top Pick for Cosmetic Injectors

AestheticRecord is an aesthetics-first platform meant for facilities that process high levels of cosmetic injectors. This platform can help bridge the gap between clinical needs and Cloud designs with an intuitive touch screen mapping system, a built-in ledger tracking for neurotoxin units, and a highly agile mobile app. The mobile app allows injectors to take before-and-after photos, attach them to a chart, and log treatment notes entirely from an iPad or smartphone.

Basically, where PatientNow fell short, and Boulevard overcompensated, AestheticRecord is kind of in the middle vis-a-vis client profiles and the data they hold. 

Now, like Boulevard or Mangomint, AestheticRecord has good options for marketing automation. One of my (handful of) issues with PatientNow was the more limited setup for marketing, and I will say that AestheticsRecord does an exceptional job supporting basic marketing functions that can scale at an enterprise level. Their software offers basic email and text marketing suites, but it’s not going to blow your mind; it’s just going to get the job done better than PatientNow. 

3. AestheticsPro: Top Pick for Data Security and HIPAA Compliance

AestheticsPro is a cloud-based, specialized medical spa software. I like that it is possibly the best alternative for clinics that need good data security and HIPAA compliance.

Honestly, if you are worried about strict state medical board oversight, and trust me I worked in states where the oversight is mind-blowing, this is a really good solution. They have really robust electronic medical records with customizable intake portals and a lot of structured sign-off loops. Basically, you can set it up so that your medical director has to sign off on basically everything.

Now, the user interface isn’t nearly as smooth as Boulevard. In fact, it’s pretty clunky if you have more tech savvy, younger generation employees. On the plus side, if you have older employees, they might find it to be right up their alley.

PatientNow’s charting environment was engineered around an older desktop-era compliance framework so it can feel a little weighty trying to click through all those drop-down menus and screen transitions. For this, I appreciate how Aesthetics Pro helps; third-party voice recognition and workflow automation tools like Fusion Narrate help keep that to a minimum. 

The downside with AestheticsPro is their online booking part of it doesn’t have a modern feel, and it can actually hurt your conversion rates with new potential clients because of how difficult it is.

4. Mangomint: Top Pick for Automation

I want to be clear that up front I do not like the color scheme or setup of Mangomint software.  

Still, color doesn’t affect function, and as far as function is concerned, Mangomint is a very intuitive setup, especially for automation and booking. Their best feature is definitely the booking flow and the billing automation; Mangomint has automated text reminders, smart payroll options, and membership payment collections. 

You really won’t notice any system lag (which gives you a lot of time to notice the tropical Barbie theme).

PatientNow was kind of a pain cost-wise, because they wouldn’t just come out and say what their costs were. I had to schedule the demo, listen to a sales pitch, etc. Eventually, one of the things that really got to me was the cost creep where I kept having to add custom reporting add-ons, integrations, and other modules. This a la carte pricing really made our overhead unpredictable. 

That’s why I appreciated Mangomint for the clear pricing structure they offered based on the size of our business. 

My biggest issue with Mangomint is it doesn’t have a lot of native medical infrastructure, so it doesn’t include things like anatomical injection mapping, clinical charting, or e-prescriptions. As such, if you are running a medical practice of any kind, you’ll have to pair this with a separate medical record system, and that can be a little complicated.

5. Zenoti: Top Pick for Enterprises 

Zenoti is a massive, enterprise-grade setup. Conflicting feelings arise when I use Zenoti. 

We had it for a while, and one part of me was really happy because it genuinely felt like the OG Microsoft Paint and Minesweeper had a love baby. On the other hand, despite how outdated and clunky it is, it actually has an absurd number of powerful corporate tools that are best suited to multi-location medspa chains, international franchises, and multi-million dollar medical facilities.

They give you centralized control over multiple international or national locations with no downtime. Things like corporate reporting, universal multi-site gift card and membership redemptions, predictive inventory tracking across dozens of warehouses, and an open API framework. 

I would be remiss if I didn’t say that with so many outstanding features comes equally outstanding complexity. The onboarding process requires a dedicated corporate training setup, and it’s really expensive because of how much time and effort goes into just understanding some of the most basic features the software has. I felt like I was drowning in PowerPoint presentations when we started onboarding, an overwhelming sick sensation that I’ve only ever experienced with the tropical Barbie colors of Mangomint. 

6. Nextech (MedSpa Edition): Top Pick for Advanced Institutions

Nextech is another enterprise healthcare EMR. This one, though, wasn’t built for international, multi-location corporations like Zenoti. It was built for plastic surgery, dermatology, and advanced medical aesthetic institutions.

If you have operating rooms and complex surgical workflows, you won’t find better clinical depth. Nextech offers compliant charts, options for impressive internal surgical scheduling matrices, and integration with specialty medical billing architectures. 

However, implementation is tricky because it is such a massive system. It has some of the same functions as Boulevard or Mangomint (think loyalty tracking and POS or basic marketing), but they aren’t easy to use, and more importantly, it doesn’t matter what else they offer if the software is just too big and too much for clients booking basic facials. 

All this to say this is going to be a great solution specifically for advanced institutions offering advanced aesthetic surgical services, but not a boutique MedSpa offering things like chemical peels.

Each of these PatientNow alternatives has something to offer, but I would only recommend Zenoti for managing a multi-location enterprise group or Nextech for surgical installations. 

For all other small-to-medium sized businesses managing one or two locations, looking to grow their customer base, and focus on consumer conversion rates, Boulevard is, in my opinion, the best option.
